We will be more than happy to submit all insurance forms for you and help you recover the most from your benefits. We will do everything we can to help you afford the treatment you need and want. For patients who require major work, a complete payment plan is designed with an appropriate payment schedule. Please call our office at Paul L. We participate with a variety of PPO INSURANCE PLANS. It is important for you to know what type of plan you have and be familiar with the coverage offered by your plan.

Dental Insurance typically covers some, but not all general dentistry procedures.

Our insurance specialist will file your claim for you, provide the necessary paperwork, and work toward maximizing your benefits.

Please remember that dental insurance is a contract between you and your insurance company, and we have no control over the benefits you receive. If you have any questions about your insurance policy, please ask! We’re happy to help you better understand your benefits.

Payment options:

• Cash or Check: Always accepted.
• Credit Card: Visa, MasterCard, Discover, or American Express.
• CareCredit: Apply online (www.carecredit.com) or in our office.

Please note: Payment is due at the time of service.

As a courtesy to you, we will bill your insurance company and track claims. Please keep us informed of any changes to your insurance plan. You are responsible for the fees charged by our office, no matter what your insurance coverage may be. Most insurance companies should respond to the claim within four to six weeks. Any remaining cost is your responsibility.
